Our objective is to empower and uplift underserved communities by providing compassionate, accessible, and culturally sensitive support through our Community Health Workers (CHW) services. We strive to bridge gaps in healthcare access, improve health literacy, and address social determinants of health ensuring individuals and families receive the resources and guidance they need to lead healthy, fulfilling lives.

Fatherhood
Parenting Initiative

The SBX Fatherhood Program offers a FREE 6-session course designed to empower fathers and strengthen their relationships with their children. Utilizing evidence-based curriculum from the Fatherhood Initiative’s 24/7 Dad, the program provides a supportive space for fathers to connect, share experiences, and learn how to raise strong, healthy children. Participants receive certificates upon completion, and opportunities to join the Fatherhood Coalition are available. Fathers play a crucial role in their children’s development, and this program aims to celebrate, uplift, and support their vital role within the family.
Rites of Passage
​Prevention and Early Intervention
​
SBX Youth and Family Services' Rites of Passage program, previously offered through the Building Resilience in African American Families (BRAAF) initiative, provides a holistic approach to empowering African American boys aged 11-13 and their families. This program blends cultural rites of passage education, and social-emotional learning, to foster resilience and well-being. Families also benefit from parent support groups and family empowerment resources. Delivered by trained youth workers, the program is designed to uplift and strengthen families.
Breathe IE
Asthma Preventative Services

The Breathe IE aims to reduce asthma-related emergencies in underserved communities of color and MediCal populations in San Bernardino and Riverside Counties. Our primary strategy involves culturally competent Community Health Workers conducting free home visits, providing asthma education, medication management, and environmental trigger mitigation.
